To get yourself started on Perform-maxx vitamins, you need to fill out the vitamin questionaire and return it to us at the Medicine Shoppe (59 Great Northern Road). Once we have received your completed form, the Medicine Shoppe staff will have a fundemental understanding of what supplementation your body needs and design a vitamin suited to your body's needs. As your body grows, develops, and changes, fill out the vitamin questionaire again and we will alter your Perform-maxx vitamin formulation for you.
